Bromelain

Bromelain refers to a group of sulfur-containing enzymes that digest protein (proteolytic enzymes or proteases) obtained from the pineapple plant. Commercial bromelain is usually derived from the stem, which differs from the bromelain derived from the fruit. Commercial bromelain contains a mixture of several proteases and small amounts of several other enzymes, and organically bound calcium. Japan, Taiwan, and Hawaii are the major suppliers of commercial bromelain.

A little more than 500 years ago Christopher Columbus discovered his first pineapple on the Caribbean island of Guadaloupe. He was intrigued not only by the fruit’s succulent flesh and aromatic juice, but also by the ways in which islanders claimed the plant could be used to heal. Bromelain was introduced as a therapeutic agent in 1957, and since that time more than 200 scientific papers on its therapeutic applications have appeared in medical literature. Many of the early studies were performed using Ananase (Rorer), an enteric-coated bromelain tablet. Later studies implied that bromelain failed in some of these early studies because of the enteric coating as well as inadequate dosages.

Bromelain Active Constituents

Bromelain (EC 3.4.22.4) is the genuine mixture of bromelain A and B, the proteolytic enzymes of pineapple fruit. The activity of bromelain is expressed in a variety of enzyme units. The Food Chemistry Codex (FCC) officially recognizes the use of milk clotting units (mcu). The gelatin-digesting unit is another enzyme unit that is acceptable and equal in numerical value to the milk-clotting unit. Different grades of bromelain are available on the basis of the milk- clotting unit. For most indications the recommended milk clotting unit range is 1,200 to 1,800.

Bromelain Typical Dosage

Unless otherwise prescribed, 80 – 320 mg of bromelain (200–800) FIP units) in 2 or 3 doses. Unless being used as a digestive aid, administration of bromelain should be made on an empty stomach. Dosage depends on the potency of the preparation.

Bromelain Side Effects

An increased tendency for bleeding in the case of simultaneous administration of anticoagulants and inhibitors of thrombocytic aggregation cannot be excluded. The levels of tetracyclines in plasma and urine are increased by simultaneous intake of bromelain. “Although no significant side effects have been noted, allergic reactions may occur. Other possible but unconfirmed reactions include nausea, vomiting, diarrhea, metrorrhagia, and menorrhagia.
